Referral Forms – Veterinarians

For referring veterinarians:

The ATC operates as a referral center for dogs and cats with mobility issues, those in need of post surgical or other rehab or those seeking integrative/alternative medicine services.  Patients will be evaluated by a licensed veterinarian or physical therapist and you will receive an evaluation report.

Our team of trained and certified staff works together with you as the referring veterinarian along with the pet owner and any other involved animal healthcare professionals to achieve the best possible outcome for the pet. We send each pet back to you for all non-therapy related treatments and concerns.

For post-surgery or neurologic cases, please send (by FAX or with the client) any relevant history,  surgery report and radiographs with the referral form.  All animals must have proof of current rabies vaccination.
