Thank you for trusting Animal Therapy Center with your patient's care. Use the referral form below to submit a referral for Medical Rehabilitation or the ATC Fitness Club. Select the appropriate referral type within the form and you will be guided through the information needed for that patient.
Medical Rehabilitation Referral
For patients being referred for veterinary rehabilitation, including post-operative recovery, mobility concerns, pain management, injury recovery, neurologic or orthopedic conditions, or other medically directed rehabilitation needs.
Fitness Club Referral
For dogs being referred for non-medical fitness, conditioning, performance support, weight-management support, recreational exercise, or general physical conditioning.
